Job summary

Surrey Downs Health and Care is seeking a Band 5 Community Nurse to work in East Elmbridge PCN. This 30-hour per week role involves delivering community nursing care and requires a current UK driving licence and access to a vehicle for work.

The post is fixed-term for 11 months. The successful candidate will contribute to holistic care planning, be able to work with diverse patient groups, and participate in staff development and training opportunities offered by the Learning & Development

Qualifications

• Registered Adult Nurse with NMC registration.
• Driving licence required to drive within the UK.
• Experience in NHS/community nursing and understanding NHS issues.

Responsibilities

• Work as part of the community nursing team to deliver holistic care.
• Assess patient needs and develop, implement and evaluate care plans.
• Maintain accurate patient records and ensure data entry is timely.
