Sussex Partnership NHS Foundation Trust is seeking a Community Nurse/OT to join the Dementia Care Home Support Team (DCHST) in West Sussex. The role focuses on improving outcomes for people with dementia, co-creating care and risk plans with service users and carers to promote independence and timely discharges.

You will work in partnership with GPs, crisis services and housing to support complex health presentations and provide specialist nursing/OT advice.

Qualifications

• Registered Nurse/OT with current registration (mental health/LD) preferred.
• Mentorship course or ENB equivalent required.
• Evidence of continuous professional development in dementia care.
• Experience conducting clinical risk assessments and managing complex health needs.

Responsabilités

• Assess, plan and implement care in the community.
• Develop care and recovery plans with service users and carers.
• Provide expert nursing/OT advice and mentor students or staff.

Connaissances

Registered Nurse/OT with current reg.
