Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is recruiting a Band 5 Community Nurse to join a friendly, supportive team delivering care in patients' homes and clinic settings.

The role offers autonomous working within a generic competency framework, with opportunities for professional development and access to support services within the Trust. Hours are 37.5 per week, with a schedule of 08:00 to 20:00, across all days of the week.

Qualifications

• Must be a registered nurse with UK NMC registration.
• Full-time, permanent post.
• Driving license and access to a vehicle are essential.

Responsibilities

• Deliver high-quality care in community/home settings.
• Conduct holistic assessments and develop care plans.
• Refer and escalate to other professionals as needed.
• Maintain accurate clinical records in line with standards.
• Communicate effectively with patients, carers and the MDT.
