Job summary

Midlands Partnership NHS Foundation Trust seeks a Band 5 Community Nurse to join the Home First Service in the north of Staffordshire. The role involves 24/7 shift patterns and travel across a large geographical area to support discharge and community care.

You will work within a multi-disciplinary team to assess, plan, and deliver evidence-based nursing care in patients' homes or clinics, promoting independence and safe discharge from hospital.

Qualifications

• Degree in nursing or equivalent.
• Registration with NMC.
• Commitment to work toward Mentorship qualification (998-997).
• Working towards Mentorship qualification (998-997).

Responsibilities

• Deliver care to patients in their homes or residential settings as part of community nursing.
• Assess and plan care for a defined population across diverse settings.
• Collaborate with multi-disciplinary teams to prevent hospital admissions.
• Support discharge planning and provide follow-up nursing care.
