Job summary

The Mid Bedfordshire Primary Home Care Team at East London NHS Foundation Trust is recruiting a permanent Community Staff Nurse to cover Sandy, Potton, Gamlingay and nearby villages. You will deliver nursing care at patients’ homes, work with a multidisciplinary team and support long-term condition self-care.

The role involves 7.5-hour shifts across seven days, 08:00 to 21:00, with travel across sites and Trust boundaries.

Qualifications

• Registered General Nurse with current NMC registration.
• Evidence-based practice and up-to-date clinical skills.

Responsibilities

• Deliver nursing care to patients in their own homes.
• Collaborate with the multidisciplinary team to plan and deliver care.
• Assess and treat complex conditions with clinical judgement.
• Support patients with long-term conditions to self-care.
• Carry out health promotion and prevention of illness.
• Participate in Urgent Response and Virtual Wards.
• Travel across sites and Trust boundaries as required.
