Job summary

West London NHS Trust is recruiting a Community Nurse to deliver high-quality care in patients’ homes within a multi-disciplinary team. The role involves travel around the local community, weekend and late shifts, and rotation through 8am–8pm services.

Newly qualified staff receive a preceptorship, clinical upskilling training, and local induction shadowing DNs, TVNs, CNS, etc. A full driving licence and car access are required.

Qualifications

• Registered Nurse with current practitioner status.
• Must hold a valid driving licence and have access to a car.
• Experience in community/home-based care is desirable.

Responsibilities

• Provide wound care, catheter care, and palliative care in patients’ homes.
• Administer IV drugs and injections, manage diabetes, and support medication administration.
• Lead and support a community nursing team within a multi-professional environment.
