CPFT Cambridge North City and Villages Nursing Team is seeking a Community Nurse (Band 5) to deliver high-quality care at patients' homes and in residential settings across Cambridge. You will develop wound, palliative, and long-term condition care, and advance your assessment and care-planning expertise.

Travel across the county is essential with a full UK driving licence and vehicle access. We value flexible, team-oriented professionals committed to excellent patient care.

Qualifikationen

• Experience of working in the community.
• Educated to RGN level.
• Commitment to high quality care and service provision.
• Able to work flexibly as part of a team.
• Ability to assess, plan and implement care.
• Able to teach and support student nurses and integrated care workers.
• Enhanced communication and interpersonal skills.

Aufgaben

• Be professionally and legally accountable for your own and delegated work.
• Follow codes of practice and professional guidelines.
• Undertake holistic assessments, including complex cases, using clinical reasoning.
• Apply evidence-based practice to ensure effective, appropriate care.
• Promote independence and reduce unnecessary hospital admissions.
• Contribute to risk and health and safety assessments and report changes.
• Communicate effectively with patients, carers and colleagues across the MDT.
• Work within the team structure and liaise with MDT and other agencies.
• Keep accurate, timely records in line with policy and guidance.
