Job summary

We are seeking a proactive Community Nurse (Band 5) to join the Cornwall Partnership NHS Foundation Trust team. The role involves delivering patient-centred nursing care in patients’ homes, coordinating with families and care teams, and ensuring safe, effective care in the community.

Successful applicants will work across varied locations in Cornwall, with flexible patterns to cover weekends/evenings as required, and opportunities for professional development within the Trust.

Qualifications

• Registered nurse with current NMC registration.
• Experience in community or district nursing is essential.
• Proven ability to work autonomously in patients' homes.

Responsibilities

• Assess, plan, implement and evaluate care in patients' home environments.
• Work with patients and carers to support independence and informed choices.
• Collaborate with Community Nurse Team leaders and locality teams; adapt to service needs.
