Royal Devon University Healthcare NHS Foundation Trust is seeking a Band 5 Registered Nurse to join the Cullompton Community Nurse Team. You will work across rural and urban community settings, providing nursing support and oversight in patients' homes throughout Cullompton, Dunkeswell and Blackdown Hills.

You will be part of an integrated MDT, with opportunities to develop skills in proactive care and early discharge. Driving is required for travel to patients’ homes.

Formación

• Must be a Band 5 Registered Nurse with current NMC registration.
• Experience in community nursing is preferred.
• Ability to assess and oversee patients in home settings.

Responsabilidades

• Provide nursing support, assessment and oversight to patients in their own homes.
• Work across Cullompton, Dunkeswell and Blackdown Hills areas with MDT input.
• Deliver patient-centred care and support discharge planning and proactive care.
