Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust is recruiting two full-time Band 5 community nurses in Carlisle. You will manage a caseload of patients with complex needs, provide specialist clinical advice, and work within a multidisciplinary team to develop and monitor care plans in patients' homes, GP surgeries, clinics, or other settings.

Formación

• Professional nursing qualification e.g. RMN/RNMH/RNLD/RGN.
• Experience of working with service users and carers in successful delivery of treatment plans.
• Complete preceptorship programme for newly qualified nurses.
• Multi-disciplinary team work.
• Proven ability to develop therapeutic partnership with service users and carers.
• Proven ability to communicate interventions to service users.

Responsabilidades

• Review and manage a caseload of patients with substantial and complex needs.
• Provide specialist clinical advice to patients, families, carers, staff, statutory agencies, third sector partner agencies and the public.
• Deliver high quality assessment and care with wider team support, advice and supervision.
• Work collaboratively with patients to develop care, recovery and wellbeing plans with regular reviews.
• Consider carer needs and involve family in care planning where appropriate.
• Deliver evidence based clinical interventions.
• Coordinate treatment planning, interventions, clinical reviews and discharge planning.
• Provide compassionate care that is based on empathy, kindness, respect and dignity.
• Incorporate a patient centred philosophy into nursing practice and patient centred collaborative care.
