Job summary

North Cumbria Integrated Care NHS Foundation Trust is seeking a Band 5 Community Staff Nurse to deliver high-quality nursing care in the Eden Integrated Care Community, spanning inpatient and community settings. You will work to prevent hospital admissions, support discharge, and manage long-term conditions through reablement.

Shifts run 8.30am–11pm on a rotational basis with weekend cover; travel across Eden Valley is required. DBS checks apply; visa sponsorship may be limited.

Qualifications

• Level 1 registered nursing qualification.
• NMC registration is required.
• Demonstrable recent nursing experience.
• Evidence of working as part of a team.
• Evidence of motivating team or peers.
• Evidence of clinical or professional development.

Responsibilities

• Provide high-quality care to patients with complex nursing needs in the community.
• Prevent hospital admission and facilitate hospital discharge.
• Promote independence through reablement.
• Manage long-term conditions.
• Work closely with health and social care colleagues.
• Work as part of the community team in the Eden Integrated Care Community.
