HCRG Care Group is seeking a compassionate Registered Nurse (Band 5) to join the Amesbury Health Centre community team. You will work autonomously within a supportive MDT to deliver safe, evidence-based nursing care and contribute to health promotion and service development.

The role offers part-time and full-time hours with NHS Pension, Agenda for Change terms, and ongoing professional development through The Learning Enterprise; a full UK driving license is preferred for community visits.

Competenze

• NMC/1st level registered Nurse with UK license.
• Full UK driving license with access to a car.
• Strong communication and organisational skills.
• Ability to work independently and in a multidisciplinary team.

Mansioni

• Participate in assessment and care planning with the Team Manager and specialist colleagues.
• Implement and evaluate care per care plan; escalate deviations to Team Manager/GP as needed.
• Communicate effectively with patients, carers and the MDT; prioritise daily work.
• Maintain accurate patient records in electronic and paper formats; support best practice.
• Participate in team meetings and daily handovers.
• Initiate health promotion activities and contribute to service development.
