Blackpool Teaching Hospitals NHS Foundation Trust is seeking a Community Nurse (Band 5) to join the WIN District Nursing Team in Poulton le Fylde. This permanent role is full-time, 37.5 hours per week, with weekend and evening work as required.

The post requires NMC Level 1 Registration (Adult Branch) and Enhanced DBS clearance. You will deliver patient-centred, holistic nursing care in the community, across homes and clinics, with strong teamwork and a commitment to NHS values.

Qualifications

• Registered Nurse level 1 with current NMC registration.
• Evidence of ongoing professional development and supervision/assessor competence.
• Continence-related qualification is desirable.

Responsabilités

• Provide nursing care across the Fylde coast in patients' homes or clinics.
• Plan, implement and evaluate care; escalate when needed.
• Monitor patient health and coordinate with families and carers.
• Administer drugs within NMC guidelines.
• Promote health and provide patient and family support.
