• Foundation Degree in Healthcare Practice as Nursing Associate.
• Experience in a Community setting is essential.

• Support the community nurse team delivering care to clients with long-term conditions.
• Assist in facilitating early discharge from hospital where appropriate.
• Provide advice and information to clients and carers to maximise independence at home.
