Job summary
Kent Community Health NHS Foundation Trust is seeking a Nursing Associate to join the Community Nursing Team, delivering high-quality, patient-centred nursing care to adults aged 18 and over in their homes.
You will work autonomously as well as within a multidisciplinary team, building therapeutic relationships to support patients with complex, long-term health conditions to remain safe, well and independent.
Responsibilities
- Undertake home visits and manage a delegated caseload.
- Participate in team handovers and multidisciplinary care.
- Deliver personalised nursing care and monitor vital signs.
- Administer medicines via oral, topical, intravenous, subcutaneous, and CMR routes.
- Support care planning and liaise with partner agencies to ensure safe, compassionate care.
