Job summary

Leaders in Care is recruiting a Community Nurse for Norfolk, East Anglia. The role offers a up to £39,000 per annum DOE with a fully expensed company car or car allowance, Monday–Friday days, and no night shifts.

You will care for adults in their own homes with a supportive MDT, manage a realistic caseload, and enjoy time, autonomy and clinical backing to deliver high-quality care.

Qualifications

  • A Registered Nurse (RGN) with an active NMC PIN.
  • Experienced in adult nursing within hospital or community settings.
  • Able to work autonomously while knowing when to seek support.

Responsibilities

  • Completing holistic assessments and developing personalised care plans.
  • Delivering safe, evidence-based nursing care and treatment.
  • Monitoring patients, evaluating outcomes and responding to changing clinical needs.
  • Educating and supporting patients, families and carers.
  • Working collaboratively with GPs and community services.
  • Maintaining accurate clinical documentation and ensuring patient safety and governance.

The Role

You'll work independently while being fully supported by an experienced multidisciplinary team, delivering high-quality nursing care to adults within their own homes.

You’ll be responsible for:

  • Completing holistic clinical assessments and developing personalised care plans
  • Delivering safe, evidence-based nursing care and treatment
  • Monitoring patients, evaluating outcomes and responding to changing clinical needs
  • Educating and supporting patients, families and carers
  • Working collaboratively with GPs, community services and multidisciplinary teams
