Job summary

Procare Community Services seeks a Registered Nurse to provide home-based community nursing care to adults, under the District Nurse, with duties including venepuncture, wound care, catheterisation, syringe drivers, end of life care and symptom control. You will work across 8am–8pm with flexible shifts and may need a UK driving licence for business use.

Successful candidates will develop competencies for potential progression to Band 6 Senior Community Nurse.

Qualifications

  • Registered Nurse with current Nursing and Midwifery Council registration.
  • Full UK driving licence and vehicle for business use if required.
  • Ability to assess nursing needs and implement care and treatment.

Responsibilities

  • Visit adult patients in their own home with identified nursing needs, including long-term conditions and end-of-life care.
  • Utilise EMIS to record care; work with iPhone/iPad for mobile working.

Job description

We are seeking a highly motivated Registered Nurse to work within a wider team of nurses and health care assistants. You will work under the direction of a District Nurse, to assess, implement and evaluate person centred care for a caseload of patients, undertaking venepuncture, wound care, catheterisation, syringe drivers, end of life care and symptom control. On achieving all relevant competencies, the post holder will be reviewed for a Band 6 Senior Community Nurse post.

Post holders will be available to work Monday to Sunday between 8am and 8pm.A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Main duties of the job

Job Purpose:o The post holder will visit adult patients in their own home who have an identified nursing need including those with a long-term condition, complex care needs and needing care at home and the end of life.o The post holder will use an electronic patient held record system, EMIS as a record of care and all our nurses are provided with a work Iphone and Ipad for mobile working.o The community nurse will use a range of nursing skills including venepuncture, wound care, catheterisation, syringe drivers, end of life care and symptom control and on successfully attaining all the competencies required will be reviewed for a Band 6 Senior Community Nurse post.

Job responsibilities

We recognise that a highly skilled community nursing workforce is the cornerstone to safe patient care and avoiding unnecessary admission to hospital and have lots training and development opportunities available, alongside a career development pathway with opportunities for development at every level through to band 3 to band 8.

All community nurses work as part of a local team, closely with the local acute hospital, social care, mental health teams, GPs, and Practice Nurses and use the electronic clinical record, EMIS with a work Ipad and Iphone provided.

  • Ability to prioritise visits in accordance with patient clinical need
  • Ability to organise own workload without direct supervision
