Job Introduction Are you a compassionate and motivated Registered Nurse looking to make a real difference in the community? We are looking for a dedicated Band 5 Community Nurse to join our friendly and supportive team. The Integrated Neighbourhood team is a community-based service which supports and works with people, either in their own home or in Residentlal or Nursing Homes. The role of the community nurse is to work with the multi professional team within community serviceproviding clinical support in assessing, planning, organising, and delivering clinical care in line with serviceand patient needs, as agreed with supervising clinicians. The community nurse works independently, working within the sphere of nursing care and within all aspectsof the nursing process. The post holder will act professionally at all times and use their knowledge andexperience to make evidence-based decisions to solve challenges and problems. They recognse and workwithin the limits of ther competencies and are responsible and accountable for their actions. The post holder will have responsibility for supervising less experienced or qualified staff and studentsduring their placements. They will be expected to actively contribute to quality assurance processes andservice development.
