Job summary

Tees Esk and Wear Valleys NHS Foundation Trust is recruiting a Community Support Worker to join the Mental Health Services for Older People Community Team in Selby. You will work with older adults and their carers, providing ongoing support, practical assistance and therapeutic interventions under supervision, in a community setting.

The role requires the Diploma in Health and Social Care Level 2 or equivalent, and literacy/numeracy Level 2.

Qualifications

• Diploma in Health and Social Care level 2 or equivalent.
• Care Certificate.
• Literacy and Numeracy level 2 or equivalent.
• Trust HCA Framework completed within 6-month probationary period.

Responsibilities

• Provide safe, patient-centred clinical activities in the community.
• Monitor mental and physical health under supervision of registered staff.
• Deliver non-pharmacological interventions to help individuals identify and achieve goals.
• Support patients and carers to monitor and review mental health needs and coordinate with services.
