Job summary

Provide support and give time to an allocated group of service users, and promote their recovery supporting their return to a community environment, or supporting them to maintain their place in the community. This is an exciting opportunity for an experienced Support time Recovery worker wishing to join the community mental health primary care team in Conwy.

Responsibilities

• Support service users to meet identified needs and outcomes.
• Assist in delivering group and individual work and clinical activity.
• Work independently and liaise with third sector agencies and carers.
• Maintain excellent links with GP practices and multidisciplinary teams.
