Job summary

Surrey and Borders Partnership NHS Foundation Trust is recruiting a mental health practitioner to provide specialist assessment, intervention and ongoing support in patients' homes and communities across the Reigate and Redhill area.

The role emphasizes care coordination, working with carers and partner agencies, and a lead professional approach for complex cases. A driving licence and NMC MH registration (or waiting) are required.

Qualifications

• NMC registration as Mental Health nurse, or waiting for registration.
• Post-qualification experience; preceptorship may be available for newly qualified nurses.

Responsibilities

• Provide specialist mental health assessment, treatment and ongoing support in patients' homes and community settings
• Undertake comprehensive mental health and risk assessments
• Deliver evidence-based nursing interventions and treatment plans
• Act as Care Coordinator or Lead Professional for people with complex mental health needs
• Work collaboratively with patients, carers, families and partner agencies
• Promote recovery-focused, person-centred care
• Contribute to safeguarding, risk management and clinical governance
• Support service development and quality improvement initiatives
• Communicate well and work within a multi-agency team
