Job summary

Surrey and Borders Partnership NHS Foundation Trust is seeking a Community Psychiatric Nurse to join the Complex Needs Elmbridge Neighbourhood Service in Molesey. You will deliver specialist mental health assessment, interventions, and ongoing support to patients in their homes and community settings, acting as a Care Coordinator where appropriate.

You will work with a multidisciplinary team, driving recovery-focused care, with opportunities to develop clinical knowledge and leadership skills.

Qualifications

• Post-qualification experience required; preceptorship may be available for newly qualified nurses.
• Current UK professional registration: RMN/RN (Mental Health) with NMC status or awaiting registration.
• Disclosure and Barring Service (DBS) check required under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
• UK registration to practise in mental health nursing is essential.

Responsibilities

• Undertake comprehensive mental health and risk assessments.
• Deliver evidence-based nursing interventions and treatment plans.
• Act as Care Coordinator/Lead Professional for individuals with complex mental health needs.
• Collaborate with patients, carers, families, and partner agencies.
• Promote recovery-focused and person-centred care.
• Contribute to safeguarding, risk management, and clinical governance.
• Support service development and quality improvement initiatives.
• Demonstrate effective communication within a multi-agency team.
