Job summary

Derbyshire Healthcare NHS Foundation Trust is seeking a Community Mental Health Practitioner in Buxton to support adults with severe mental ill health. The post emphasizes person-centred care, collaboration within a multi-disciplinary team, and delivering recovery-focused services across a wide area.

You will hold a relevant professional qualification (MH Nursing, SW or OT) and bring post-registration experience with CPA, risk assessment, and care planning.

Qualifications

• Must hold a Mental Health nursing, social work or occupational therapy qualification.
• Experience with Care Programme Approach (CPA).
• Post-registration experience in mental health settings.
• Ability to travel across a wide geographical area to support service users.

Responsibilities

• Work with people experiencing severe and enduring mental ill health to enable recovery.
• Manage a caseload and support team duties.
• Apply CPA knowledge and demonstrate risk assessment in community settings.
• Provide mentoring and supervision to staff and students.
• Engage in supervision and training opportunities within the team.
