Job summary

Nottinghamshire Healthcare NHS Foundation Trust is seeking a Band 6 Mental Health Practitioner/Community Mental Health Nurse to join the Bassetlaw Local Mental Health Team in a community setting. The role focuses on comprehensive assessments, personalised recovery planning, and complex case management within a multidisciplinary team.

You will be supported with training and development opportunities, and the team operates Monday to Friday, 9am–5pm.

Qualifications

• Professional Qualification (Mental Health) to degree level or equivalent.
• Maintains current registration with the NMC or equivalent registration.
• Practice based educator qualification or equivalent (or prepared to undertake training).
• Further specialist knowledge/post registration experience in specific service area or other formal training.
• Demonstrate continued professional development to maintain professional registration.
• Evidence of post registration/specialist qualification in specific service area or other formal training.

Responsibilities

• Caseload management under the Care Programme Approach.
• Initial assessments and one-to-one interventions with service users.
• Undertake and record comprehensive need/risk assessments, devise and implement treatment plans for service users.
• Demonstrate application of specialist skills and knowledge to maintain professional competence and fitness to practice as a nurse practitioner.
• Establish and maintain robust communication networks with clients, carers, and other health care workers within urgent care and the community.
• Use interpersonal skills to diffuse emotive situations during assessments and pathway planning within acute settings.
• Demonstrate high level communication skills.
