Responsibilities

• Caseload management under the Care Programme Approach
• Initial assessments and one-to-one interventions with service users
• Record comprehensive need/risk assessments, devise and implement treatment plans for service users
• Demonstrate application of specialist skills to maintain professional competence and fitness to practice as a nurse practitioner
• Establish and maintain robust communication networks with clients, carers, team members, other health and social care workers within urgent care and the community
• Use interpersonal skills to diffuse emotive situations while undertaking assessments and pathway planning within acute settings
• Demonstrate high level communication skills
