Job summary

• Must demonstrate ability to manage caseloads in community settings.
• Ability to perform comprehensive needs/risk assessments and devise treatment plans.
• Strong communication with clients, carers and multi-disciplinary teams.

Responsibilities

• Caseload management for patients living in the community as care coordinator.
• Triage referrals and undertake initial assessments.
• Record comprehensive need/risk assessments and implement treatment plans.
• Establish and maintain channels with clients, carers and other health/wellbeing professionals.
• Possess driving licence and vehicle for business use; adjustments available under Equality Act 2010.
