Job description

The post-holder will work as a member of the NLFT Homeless Health Service in partnership with NLFT and relevant stakeholders.

The role is part of an initiative to improve care for the rough sleeping population in NCL boroughs by:

  1. Providing a responsive service to identify mental and physical health issues.
  2. Collaborating with other services to deliver timely care and interventions based on clinical needs.
  3. Supporting individuals to move off the streets and maintain a settled lifestyle.
  4. Referring service users to appropriate services and supporting them through the process.
  5. Ensuring all service users are registered with a GP.
  6. Working within an MDT as a member of the NLFT Homeless Health Service.
  7. Providing healthcare in non-traditional settings, including on the streets.
  8. Maintaining professional curiosity.
  9. Liaising with agencies to plan and assess care needs.
  10. Providing clinical supervision and caseload management to junior staff.
  11. Extending clinical and managerial expertise as delegated, including deputising for the Senior Practitioner when required.
  12. Conducting risk assessments and delivering holistic care to homeless individuals with complex needs.
  13. Partnering with GPs, consultants, nursing and social care colleagues, outreach teams, and housing support services to treat various mental health conditions.
  14. Referring service users to other team members, secondary care, and statutory or voluntary organizations as needed.
  15. Providing evidence-based care aligned with clinical guidelines, protocols, and professional standards.
