Job description

The post-holder will work as a member of the NLFT Homeless Health Service in partnership with NLFT and all relevant stakeholders.

The post is part of an initiative to improve the quality of care available to the rough sleeping population in NCL boroughs through:

  1. Providing a responsive service that identifies mental and physical health problems.
  2. Collaborating with other services to deliver care and interventions based on clinical need at the right time.
  3. Providing interventions to support people to move off the streets and sustain a more settled lifestyle.
  4. Referring service users to the most appropriate service and supporting them until accepted.
  5. Ensuring all service users are registered with a GP.
  6. Working within MDT as a member of the NLFT Homeless Health Service.
  7. Carrying out health care responsibilities with clients in non-traditional settings, including on the streets.
  8. Maintaining professional curiosity.
  9. Liaising with other agencies to plan and assess care needs.
  10. Providing formal, ongoing clinical supervision and caseload management supervision to junior and less experienced community mental health staff, as directed.
  11. Undertaking duties to extend clinical and managerial expertise as delegated by the manager, including deputising in the absence of the Senior Practitioner.
  12. Conducting risk assessments and needs assessments with expert clinical skills to deliver holistic care to people who are street homeless in Barnet, Enfield, and Haringey, who may find it difficult to access mainstream services.
  13. Partnering with GPs, consultants, nursing and social care colleagues, outreach teams, and housing support services to treat service users with a range of mental health conditions and complex needs.
  14. Referring service users to other team members, secondary care, and statutory or voluntary organizations as needed.
  15. Providing and promoting evidence-based care in line with clinical guidelines, protocols, and professional codes of conduct.
