Job summary

Thurrock Council is seeking an exceptional Support Planner for the Community Led Support (CLS) Team 2. The role focuses on connecting vulnerable residents with their communities and delivering person-directed care that looks at what matters.

You will collaborate with teams, health partners and agencies to meet care needs while safeguarding the people we serve. We value flexible, hybrid working and a supportive environment.

Qualifications

• Excellent communication and ability to work with colleagues across departments, health and community partners.
• Experience in Health & Social Care settings is essential.
• Understanding of the social model of disability and person-directed care is required.
