Job summary

Salford City Council is recruiting for an experienced social care practitioner to provide assessment, support planning, monitoring and review for people and carers. You will work within a busy Intermediate Care Social Work team and collaborate with a multi-disciplinary team to secure positive outcomes.

You will support individuals leaving the Intermediate Care Unit or completing reablement with our Home First service, using a person-centred approach and clear, compassionate communication.

Qualifications

• Experience working in Adult Social Care.
• Familiarity with legislation and guidance underpinning the work.
• Ability to deliver sensitive information compassionately.

Responsibilities
