Job summary

Swindon Borough Council is recruiting a Social Care Practitioner to join the Community Led Support West Team. The role focuses on strengths-based conversations, assessing needs and developing support plans to promote independence and wellbeing.

You will work with adults, carers and professionals, travel across the borough, and operate a hybrid model with office presence two days a week. An enhanced DBS check and valid NVQ3 are required.

Qualifications

  • Has an NVQ Level 3 in Health and Social Care (or equivalent).
  • Is passionate about delivering person-centred, strengths-based practice.
  • Has experience of working within Adult Social Care, health, or a related setting.
  • Possesses excellent communication and interpersonal skills.
  • Is able to build positive relationships with individuals, carers and professionals.
  • Can manage competing priorities in a fast-paced environment.
  • Has knowledge of adult social care legislation and safeguarding principles.

Responsibilities

Please note: 3.3% national pay award increase to be applied in October.

Your next role

We have an exciting opportunity for a Social Care Practitioner to join our friendly, supportive and dynamic Community Led Support (CLS) West Team within Adult Care. This is an excellent opportunity for an enthusiastic and motivated practitioner who is passionate about strengths-based practice and empowering individuals to achieve their outcomes.

About the Role

Our aim is to understand people's needs, maximise independence and, where possible, resolve queries and issues quickly and effectively through strengths-based conversations.

Working within the team, you will:

  • Hold strengths-based conversations with adults, carers and families.
  • Explore community-based solutions and local support networks.
  • Provide information, advice and guidance to help people make informed choices.
  • Complete assessments and reviews in line with the Care Act.
  • Identify outcomes and develop support plans that promote independence and wellbeing.

We are looking for someone who:
  • Has an NVQ Level 3 in Health and Social Care (or equivalent).

You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act "for the effective performance of a customer-facing role").

The ability to travel throughout the Borough will be required and access to a car to undertake home visits.

The team operates a hybrid working model with all practitioners expected to be in the office for a minimum of two days per week.

You will be subject to an enhanced Disclosure and Barring Service Check.
