Job summary

Thurrock Council is seeking an exceptional Support Planner for the Community Led Support (CLS) Team 2. The role focuses on connecting vulnerable residents with their communities and delivering person-directed care that looks at what matters.

Qualifications

  • Excellent communication and ability to work with colleagues across departments, health and community partners.
  • Experience in Health & Social Care settings is essential.
  • Understanding of the social model of disability and person-directed care is required.

Job description

Thurrock Adult Social Care are seeking an exceptional individual to join us as a Support Planner in our Community Led Support (CLS) Team 2. This role is an interesting opportunity to work in a team that is committed to connecting individuals with their communities. The team is about responsive social care support that creates opportunity for vulnerable people to come together with their community to explore opportunities and outcomes, while providing support that is committed to safeguarding the most vulnerable. As part of a CLS team you will be working in a way that is responsive and proportionate, working with people to achieve their outcomes using their strengths and their community creativity, linking people with their communities focusing on 'what matters rather than what's the matter'. What We Are Looking For You will need to be an excellent communicator and able to work alongside colleagues in other departments, health, community partners and agencies in order to meet the care and support needs, and outcomes for our residents. You will need to have an understanding of the social model of disability and the concept of person-directed care or the delivery outcomes to adults. Experience of working in a Health & Social Care setting is also essential for this role.

A Social Care qualification NVQ level 3 or recognised Social Care or Health qualification is essential for this role.
