Job summary

Thrive Tribe is recruiting a Community Lead for Smokefree Island on the Isle of Wight. You will lead a team of practitioners delivering evidence-based smoking cessation support and drive performance across the locality.

You'll balance day-to-day leadership with service improvement, using data to identify opportunities, improve referrals and outcomes, and collaborate with GP practices, pharmacies and NHS partners. A passion for health equity and people development is essential.

Qualifications

• Experience leading people and delivering results.
• Ability to build relationships and collaborate with partners to improve access and outcomes.
• Data-driven approach to performance and service improvement.

Responsibilities

• Lead a high-performing team of practitioners delivering smoking cessation support.
• Drive service performance against contractual objectives and KPIs.
• Ensure high-quality, evidence-based, person-centred care.
• Build and maintain partnerships with GP practices, pharmacies, NHS partners and community organisations.
