Job description

Serve as the main point of contact for processing referrals into community services. Handle a high volume of calls from various agencies in a busy office environment. Process referrals and associated correspondence promptly and accurately. Follow up on queries and ensure patient records are updated efficiently. Utilise decision-making skills to signpost service users and professionals to appropriate service areas. Facilitate effective communication and collaboration across professional and organisational boundaries.

Working Hours

The Hub operates from 8am to 5pm, Monday to Friday, with shift patterns within these times. This role includes occasional weekend hours to manage a small number of referrals received outside normal working hours. Weekend hours are shared within the team and occur no more than once per month.
