Job summary

Leicestershire Partnership NHS Trust is seeking a Community Hub Coordinator to support nursing and therapy teams across community settings. The role requires flexibility to work across St Luke's Hospital, Market Harborough and Springfield Road Health Centre in Leicester, travel between sites, and extended hours with enhanced shifts.

You will process referrals, manage tasks, support audits and data entry, and operate the SystmOne system.

Qualifications

• GCSE English and Maths at grade C or above.
• NVQ Level 3 in Business Administration or equivalent.
• Experience in the NHS or a similar complex organisation.
• Background in a customer service environment.
• Experience handling varied and changing administrative demands.

Responsibilities

• Process referrals and manage tasks for the Community Nursing and Therapy teams.
• Support data collection for audits and ensure accurate data entry.
• Use the electronic patient record system (SystmOne).
• Order and monitor equipment and supplies; work across sites as needed.
• Be highly organised, a strong team player with excellent communication.
