West London NHS Trust is seeking a staff nurse for community physical health services. The role involves home visits delivering wound, catheter, palliative and diabetes care, with IV/injection administration and potential team leadership.

Shifts include early/late, weekends and bank holidays, and a full driving licence with access to a car is required. Join a multidisciplinary team of district nurses, therapists and health care assistants to coordinate care pathways with local authorities, GPs

Competenze

• Registered General Nurse, adult branch
• Evidence of teaching and assessing, such as preceptorship or mentorship, or working towards it
• Evidence of recent CPD
• Experience in community care preferred

Mansioni

• Deliver nursing care to patients in their homes, including wound, catheter, palliative and diabetes care
• Administer IV drugs, medication and injections
• Contribute to leading a community nursing team within a multi-professional environment
• Work within an integrated team of district nurses, specialist nurses, community matrons, therapists and health care assistants
• Work with local authorities, Integrated Care Networks, GPs and acute providers to build care pathways that support early discharge and prevent admission
• Support colleagues to develop new skills and advise patients' families
• Travel around the local community
