Job summary

Rutland County Council in Rutland is seeking a Registered Nurse to join the MiCare service, delivering person-centered, preventative care in people’s homes across the county. This NHS Band 5 equivalent role requires strong clinical and collaborative skills to support independence and prevent unnecessary hospital admissions.

Qualifications

• Registered Nurse (NMC) qualification.
• Experience in community health or social care settings.
• Strong communication and collaboration skills.
• Ability to train and mentor staff on clinical tasks.
• Commitment to person-centred care and prevention-focused practice.

Responsibilities

• Support the development of a neighbourhood and prevention agenda through a MDT approach.
• Provide clinical advice and support to the MiCare Team as required.
• Deliver training on delegated healthcare tasks to staff.
• Build effective links with GP surgeries and community nursing teams to ensure a holistic approach for individuals using the service.
• Contribute to care planning that prioritises independence and well-being.
