Cromwell Medical Staffing is seeking a Band 5 Community Worker to support people across the East of England. You will help individuals maintain independence at home and access the services they need within their communities.

You will carry out assessments, contribute to person-centred care plans, work with healthcare professionals and families, promote wellbeing, keep accurate records, and follow safeguarding and confidentiality standards.

Qualifications

• Minimum 6 months NHS or community healthcare experience.
• Experience working with vulnerable adults, families, children, or community services.
• Good organisational and time management skills.
• Understanding of safeguarding and confidentiality.
• Full UK driving licence and access to a vehicle.

Responsabilités

• Support service users with their health, wellbeing, and social care needs.
• Carry out assessments and contribute to person-centred care plans.
• Work closely with healthcare professionals, families, and partner agencies.
• Promote independence and positive health outcomes.
• Keep accurate and timely records.
• Follow safeguarding, confidentiality, and professional standards.
• Participate in team meetings, supervision, and training.
