Buckinghamshire Healthcare NHS Trust invites applications for an RRIC Community Practitioner - Physiotherapist (Band 6) in Amersham. The post covers rapid response, community physiotherapy and intermediate care with 37.5 hours per week, 8 am–6 pm, weekends included. Flexible working is available, with a salary of £39,959–£48,117 per year.

Formación

• Registered Healthcare Professional with HCPC or RCN registration.
• Degree/ Diploma in Physiotherapy, Occupational Therapy, Nursing or Paramedic Science.
• Experience of recent Continuous Professional Development.
• Postgraduate clinical training in areas such as respiratory, cardiac, orthopaedics and end of life care.

Responsabilidades

• Triage and comprehensive specialist assessment of patients and carers.
• Plan programmes of care to promote health gain and independence.
• Allocate workloads within the Rapid Response & Intermediate Care team.
• Collaborate with a range of professionals to ensure timely and appropriate care and lead on care planning.
