South Tyneside and Sunderland NHS Foundation Trust is seeking a Band 4 Assistant Practitioner to join the District Nursing (DN) Teams in South Tyneside. You will carry out planned and unplanned nursing interventions in patients’ homes, residential and care settings, under supervision and independently as appropriate.

You will support registered nurses, work within Trust policies, and help deliver high-quality, compassionate care that respects patient privacy and dignity.

Qualifications

• Diploma or equivalent appropriate qualifications (UK equivalent), e.g. foundation degree; or NVQ3 level qualification plus short courses or relevant experience to diploma level
• Substantial experience of working in a Health and/or Social Care setting
• Experience of working collaboratively with a wide range of staff
• Working knowledge of National Policies/Procedures across Health and Social Care and Local Implementation

Responsabilités

• Work within Integrated Care Team, providing care in patients' own homes / care settings
• Carry out planned and unplanned nursing interventions as designated by the lead clinician
• Provide advanced support to registered nurses and follow Trust policies and protocols
• Work flexibly across care boundaries to support patients in homes and care homes
• Comply with Trust values and behavioural compact
• Ensure safe, caring environment with privacy, dignity and respect for patients
