Job summary

NELFT NHS Foundation Trust is seeking a Band 4 Senior Rehabilitation Assistant to join the Falls Service, delivering community rehabilitation and falls prevention. You will assess and treat patients, progress tailored plans, and provide education to patients, carers and staff under supervision when needed.

A car driver is required to travel to Barking and Dagenham, Havering and Redbridge for home visits and group sessions, upholding dignity and safeguarding, while maintaining accurate records

Qualifications

• Car driver with ability to travel to Barking and Dagenham, Havering, Redbridge.
• Experience in rehabilitation and falls prevention.
• Excellent communication and clinical reasoning.
• Ability to work autonomously with supervision as needed.

Responsibilities

• Deliver delegated therapy and nursing interventions.
• Progress treatment plans and monitor patient outcomes.
• Educate patients, carers and staff.
• Promote safe moving and handling and use of adaptive equipment.
• Maintain clinical records and ensure consent and privacy.
