Job summary

Betsi Cadwaladr University Health Board is seeking a Band 5 Community Nurse to join the Caia Park District Nursing Team in Caia Park, Wrexham, working 37.5 hours per week. The post requires NMC registration with ongoing CPD and the ability to work both independently and within a team.

The successful candidate will assess, plan, implement and evaluate care in the community, liaising with other agencies to provide clinically effective care. Welsh language skills are desirable.

Qualifications

• NMC registered nurse with ongoing CPD.
• Experience in community/family nursing desirable.
• Ability to document care clearly and accurately.
• Good IT skills and basic computer literacy.

Responsibilities

• Assess, plan, implement and evaluate care in the community.
• Work as part of the district nursing team and with other agencies.
• Provide clinically effective care and maintain patient safety.
