Job summary

Aneurin Bevan University Health Board is seeking an experienced Community Staff Nurse (Band 5) to join the Bargoed district nursing team. The post covers care in patients' own homes, with a commitment to high standards and collaboration within the locality.

The role is 37.5 hours per week, permanent, offering a supportive team and ongoing professional development within a busy community services environment in Wales.

Qualifications

• Meets all essential criteria as per attached job description/person specification
• Meets all desirable criteria as per attached job description/person specification
• To work in the UK, all candidates who are not UK or RoI nationals require sponsorship to obtain either a Health and Care visa or Skilled Worker visa

Responsibilities

• The district nursing service is fundamental in the delivery of the health boards’ clinical futures model in providing care closer to home enabling early discharge and preventing hospital admissions.
• We deliver holistic care to patients within their own homes and residential homes which meets the increasing complexities in health of our patient population.
• Assessment, planning and delivering high standards of care are achieved through effective collaboration and partnership working within our locality.
