Job summary

Qualifications

• First level registered nurse (Parts 1 or 12) required.
• Excellent IT skills essential.
• Good oral communication skills; fluent in English.

Responsibilities

• Screening referrals and triaging them.
• Providing planned and unplanned care to patients in the community.
• Carrying out complex holistic assessments.
• Providing palliative care.
• Working to avoid unnecessary hospital admissions.
• Providing catheter care and bowel management.
• Providing wound management.
• Care planning for future visits with patients and carers.
• Liaising with community nurse team leaders and GPs.
