Essex Partnership University NHS Foundation Trust is seeking a compassionate Community Registered Nurse to join the District Nursing Team serving Castlepoint. The role requires a full UK driving licence and access to a car, with bank holidays and weekend coverage as part of a 7-day service.

You will screen referrals, provide planned and unplanned care in the community, and work closely with GPs and MDT members.

Qualifications

• Registered Nurse (Parts 1 or 12).
• Full UK driving licence and access to a car for work purposes.
• Demonstrates ability to plan and prioritise workload.

Responsabilités

• Referral triage (assessing and prioritising referrals).
• Complex holistic assessments.
• Hospital avoidance and palliative care planning.
• Catheter care and bowel management.
• Liaising with MDT/PCN and carers.
• Wound management and care planning for future visits.
• Excellent IT skills and documentation.
