ESSEX PARTNERSHIP UNIVERSITY NHS FOUNDATION TRUST is recruiting for a Community Nurse Team Leader to assess care needs, plan and implement programmes of care, and lead clinical nursing management across a geographical area, including weekends and evenings. Qualifications

• Undertake comprehensive nursing assessments to identify physical and environmental needs.
• Plan, evaluate and prioritise interventions and care.
• Lead clinical nursing management of the most dependent patients.
• Work without direct supervision and cover weekends/evenings.

Responsibilities

• Takes responsibility for a patient or defined group within own home or elsewhere.
• Undertake comprehensive nursing assessments to identify needs.
• Plan, evaluate and prioritise interventions and care.
• Lead clinical nursing management of the caseload's most dependent patients.
• Review caseload patient care and inform Senior Nurse of changes.
• Establish workload priorities and manage time efficiently.
• Act up in Senior Nurse's absence.
• Deliver nursing care across a geographical area including weekends/evenings.
• 27 days holiday, plus bank holidays, rising to 33 days after 10 years' service.
• Excellent pension of up to14.5% of your pensionable pay.
