Job summary

The post is a Community Nurse role within the District Nursing team at Somerset NHS Foundation Trust, open to newly qualified and experienced nurses. The work involves managing a caseload, making autonomous clinical decisions, and building holistic care plans with Primary Care Networks.

You will liaise with Neighbourhood Teams, support 8am–10pm shifts across seven days, and may have a preceptorship for newly qualified nurses. A car/own vehicle is required for patient visits.

Qualifications

• Relevant degree in Nursing or equivalent qualification.
• Registered nurse or newly qualified.
• Coaching/mentorship training preferred.

Responsibilities

• Carry out holistic assessments and develop care plans.
• Make autonomous clinical decisions and delegate care where appropriate.
• Manage a caseload and oversee patient care pathways.
• Administer medications, including IV medications.
• Provide palliative and end-of-life care and wound management.
• Liaise with Primary Care Networks and Neighbourhood Teams.
