Job summary

Pertemps Medical is seeking a Band 5 District Community Nurse for ongoing agency shifts across the West of England. You will deliver high-quality community nursing care, assess patients, manage plans, administer medications and support discharge planning.

This role offers flexible shift patterns, block bookings in advance and reliable ongoing work with competitive weekend and Bank Holiday rates.

Qualifications

• Active NMC registration.
• Experience in district or community nursing.
• Strong assessment and care-planning skills.
• Experience managing own caseload.
• Good communication and MDT working skills.
• Ability to work autonomously in the community.

Responsibilities

• Completing patient assessments and developing individualised care plans.

• Continence care.
• Supporting patients with long-term conditions.

What We’re Looking For

We are looking for experienced Band 5 Community Nurses who are confident working independently within a community setting.

• Previous District or Community Nursing experience
• Strong assessment and care-planning skills
• Experience managing your own caseload
• Good communication and MDT working skills
• Ability to work autonomously in the community
