Job summary

Discovery in Yeovil is seeking a Community Connector to support individuals with learning disabilities and autism to live independently and engage with their local community. You will help people discover interests, connect with opportunities, and become active, valued members of their communities.

The role involves building relationships with families, employers and local groups, coordinating activities from visits to fitness sessions, volunteering and work placements.

Qualifications

• Full UK driving licence and access to a car.
• Experience supporting people with learning disabilities or autism is desirable.

Responsibilities

• Support around 10–11 people with learning disabilities and/or autism to live the lives they choose.
• Help people access meaningful activities, employment, volunteering and social groups within the community.
• Build strong relationships with individuals, families, employers and community groups to create opportunities.
