Discovery Inc is seeking a Community Connector to support 24 adults with learning disabilities and/or autism across Street and Yeovil. You will help individuals access activities, relationships, volunteering, employment and local resources while promoting independence.
The role involves some lone-working, travel across sites, and shifts between 8:30am and 10:00pm, including evenings and weekends. A full UK driving licence and own vehicle are required, with DBS training provided.
Are you passionate about helping people build meaningful connections, achieve their goals and become active members of their local community? As a Community Connector, you\'ll support 24 adults with learning disabilities and/or autism to live the lives they choose, focusing on their aspirations, independence and community involvement. You\'ll work closely with the people we support to identify what matters most to them, helping them access opportunities, activities, relationships, volunteering, employment and community resources that enhance their quality of life. Based across our services in Street and Yeovil, you\'ll provide primarily face-to-face support, building strong relationships with the people we support, their families, colleagues and wider community networks. This is a lone-working role where you\'ll need to be confident working independently, using your initiative to problem-solve, communicate effectively and respond flexibly to changing needs. You\'ll support people with day-to-day travel arrangements, maintain accurate records using IT systems, and promote choice, control and independence in every aspect of support. You\'ll be supporting 24 adults with mixed levels of support needs, including providing moving and assisting support where required, including the use of walkers. Personal care and medication support may also form part of the role when needed. Shifts: You\'ll work flexibly across our Street and Yeovil locations, with shifts operating between 8:30am and 10:00pm, including evenings and weekends. Travel and Drivers license: As community-based travel is an important part of the role, you\'ll need to hold a full UK driving licence, have access to your own vehicle, and be willing to use it for work purposes with appropriate business insurance in place. If you\'re someone who enjoys meeting new people, building community connections and empowering others to achieve their ambitions, this is an opportunity to make a genuine difference every day while helping people live full and meaningful lives.
